I, Catherine Clark Cummings (widow), a naturalized and loyal citizen of the United States, hereby apply to the Department of State, at Washington, for a passport


I solomnly swear that I was born at Aberdeen, Scotland on or about the 27 day of March, 1879, that my husband emigrated to the United States, sailing on board the Maridian from Glasgow on or about October 1909; that he resided 7 years, uninterupted in the United States, from 1909 to 1916 at Dorchester, Mass.; that he was natuarized as a citizen of the United States before the District court of U.S.A at Boston, Mass, on 26th April, 1915 a shown by the accompanying Certificate of Naturalization; that I am the widow of the person described in said Certificate; that I have resided in the United States, uniterruptedly, for 6 years from 1910 to 1916, at Dorchester, Mass; that I am domiciled in the United States, my permanent residence being at 12 Washington Street, in the State of Mass, where I follow the occupation of attendant; that I am about to go abroad temporarily; and indent to return to the United States within indefinite months/years with the purpose of residing and performing the duties of citizenship therein; and that I require a passfor for use in visiting the countries hereinafter named for the following purpose:

To join my children and make my house there near my parents.


I intend to leave the United States from the port of New York on board the American Line on Mar 15 to April 1 1918
