District of Massachusetts, In the District Court of the United States,

I, Annie Constance Ross Mitchell, aged 37 years, occupation Nurse, do declare on oath that my personal description is: Color white, complexion fair, height 5 feet 6½ inches, weight 160 pounds, color of hair brown, other visible distinctive marks none.

I was born in Embo, Sutherland, Scotland on the 28th day of November anno Domini 1890 ; I now reside at 661 Rogers St.Lowell, Massachusetts.

I emigrated to the United States of America from Glasgow, Scotland on the vessel SS Celtic; my last foreign residence was Embo, Scotland; I am married; the name of my husband is Thomas; he was born at Arbroath, Scotland and now resides with me.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentiate, state or sovereignty, and particularly to George V, by the Grace of God, of Great Britain, Ireland and the British Dominions beyond the Seas, King, Defender of the Faith, Emperor of India of whom I am now a subject; I arrived in the port of New York in the State of New York, on or about 7th day of May, anno Domini 1920; I am not an anarchist; I am not a polygamist nor a believer in the practice of polygamy; and it is my intention in good faith to become a citizen of the United States of America and to permanently reside therein: So help me God.

at Boston, Mass., this 5th day of March anno Domini 1928.
