I, Donald Cumming now residing at 82 Floral St, Newton Highlands, Middlesex Co, Mass.

Occupation Painter, aged 45 years, do declare on oath that my personal description is:

Sex male, color, white, complexion light, color of eyes blue, color of hair light brown, height 5 feet 8 inches; weight 150 pounds; visible distinctive marks none; race Scotch; nationality British.

I was born in Embo, Sutherland, Scotland, on Oct 11 1892.

I am married. The name of the wife is Jessie.

We were married on April 12, 1918 at MIddlesborough England; she was born at Embo Scotland, on Jan 8 1893, entered the United States at New York, New York, on Mar 1 1921, for permanent residence therein, and now resides at Newton Highlands. I have 2 children, and the name, date, and place of birth and place of residence of said children as as follows:

Subscribed and sworn before me in the form of oath shown above in the office of the Clerk of Said Court, at Boston Mass this 20th day of August, anno Domini, 1938 certification no 1-231750 on the Commisioner of Immigration and Naturalization showing the lawful entry of the déclarant for permanent residence on the date stated above, has been received by my. The photograph affixed to the duplicate and triplicate hereof is a likeness of the déclarant.
